Driver Screening - Driver Screening over 70 years of age

Please complete and turn in to be cleared and registered with insurance company to drive church vehicles.

A form must be completed for all requests and given to Dana Roberts. She will be responsible for making sure it gets to the appropriate department and that the request is completed in a timely manner.
